Pegloticase is indicated when:

A. A patient is first diagnosed with gout.

B. A patient's serum urate level is above 6 mg/dL (360 µmol/L).

C. Previous ULTs have failed to significantly reduce a patient's serum urate level and degree of pain.

D. A patient with gout does not adhere to recommendations of limited alcohol consumption and modified dietary intake.
